Hospital Partners

Hospitals Sign On to Reduce Avoidable Readmissions.

The following hospitals have signed on to support the No Place Like Home Campaign and reduce overall readmissions by 20 percent from their 2010 baseline—as measured by HSAG—by June 30, 2013. The Supporting Partners started recruiting hospitals in January 2012. New hospitals will be listed as they join.

Their commitment underscores the leadership that's needed to significantly reduce avoidable readmissions—an issue that will require collaboration among community stakeholders. Reaching our goals will enable us to achieve the Triple Aim of improving population health, the patient care experience, and the affordability of care in this area.
