Hospital Participation

The No Place Like Home Supporting Partners are seeking commitment from all Arizona hospitals to participate in the No Place Like Home Campaign, particularly those hospitals that account for approximately 80 percent of the annual hospital readmissions. Participating hospitals commit to meet aggressive but realistic goals and to partner with their local care delivery organizations to achieve them.

Participants in the Campaign will include hospital-led teams of interdisciplinary representatives from the hospital, associated clinics, home health agencies, nursing homes, and other key stakeholders. All of the teams will share a commitment to participate and a willingness to devote staff and resources to No Place Like Home efforts.

The success of this Campaign for hospitals and individual care providers depends on the mutual commitment and collaborative efforts of both the Campaign Supporting Partners and the participating organizations.

Participation Requirements

While there is no fee to participate, organizations joining the Campaign must commit resources and staff to the initiative in order to meet the following expectations.

  1. Reduce overall readmissions as measured by CMS claims data by 20 percent over the 2010 baseline data between January 1, 2012, and June 30, 2013.
  2. Conduct an organizational assessment (hyperlink please) of the hospital's readmissions. Visit the Resources section of the site for more information.
  3. Share the organizational assessment results and readmissions data with Health Services Advisory Group, Inc. (HSAG).
  4. Commit to improving performance in key areas identified through the organizational assessment.
  5. Agree to publicly disclose participation in the Campaign.
  6. Have the CEO sign and fax back the Campaign Organizational Commitment.

Helping Hospitals Succeed

The Campaign's supporting partners—HSAG, APIPS, ADHS, and AzHHA—will support Arizona hospitals and their community partners to achieve the Triple Aim goals during the 18-month No Place Like Home Campaign. Participating hospitals will receive support to implement the interventions most likely to accelerate their work and achieve success.

Participant benefits
Participants will receive a variety of services and resources from the Campaign, including:

  1. Opportunities for participating organizations to come together for collaborative learning, planning, and networking and provide resources via the No Place Like Home Web site.
  2. Monthly education offerings that feature local and national organizations and profile best practices being used, innovative experiences, and experts in the field to reduce readmissions.
  3. Technical assistance as you conduct a readmission root cause analysis (RCA) and develop an Action Plan for reducing avoidable readmissions based on the RCA results.
  4. A No Place Like Home resource consultant to serve as the lead contact person for each participating organization and to provide ready access to program resources. If you are unsure who your resource consultant is, contact Cheryl Angotti at cangotti2@hsag.com or 602.801.6916.
  5. A strong commitment to leverage opportunities to advance the project through partners and stakeholders and to promote the accomplishments, lessons learned, and successes of participant communities.
  6. Organizational assessment assistance with each organization's team at the beginning of the project, review of measures developed, and an evaluation at the end of the project, upon request.
  7. An online library of resources and toolkit materials to assist organizations with readmission RCA, as well as provide evidence-based/best practice interventional materials, measurement tools, and communication materials for internal and public promotions, just to name a few.
  8. Ongoing updates to participating communities as new resources and tools become available.

Not Ready to Start Immediately?

Hospitals that are most ready to focus on one or more of the five areas will begin in January 2012. Hospitals less ready at the onset can join the campaign over time.
