2012 Regional Learning and Action Collaboratives
Since so many of us are working to reduce avoidable hospital readmissions, doesn't it make sense for communities in specific geographic locations to meet on a periodic basis to share effective strategies and lessons learned?

Objectives:

  • Convene meetings specific to geographic regions for all providers that are actively developing strategies and tactics to reduce avoidable readmissions.
  • Share promising practices in areas impacting avoidable readmissions, with permission to "steal shamelessly" from each other.
  • Develop community networking relationships that include those individuals who are working on reducing readmissions.
